Credit Application

To assist our customers in applying for credit for open account purchases, Wilkerson Instrument Co., Inc. is happy to provide a credit application on our website.  The credit application is provided in both Microsoft® Word and Adobe® PDF formats for our customer convenience.

To apply for credit:

  1. Download the appropriate file for your use.

  2. Complete all requested information.  (Or attach your company credit information letter and complete the company information and any information not provided on your company letter.)

  3. Sign and date the application.

  4. Attach any additional documents required by your state. ie;

    Resale Certificates
    Tax Exempt Certificates
    Direct Pay Authorizations

  5. Fax the completed form to 863-644-5318.
